As part of the measures to contain the spread of COVID-19, the Office of Student Affairs initiated online modes of delivering our services through the use of Google Forms.
Selected services will now be done online.

A confirmation receipt will be sent via email and is required to be presented to the attending personnel otherwise transaction will not be accommodated.
